Factors Affect the Prognosis of Lupus Nephritis

Kidney involvement becomes clinical symptom of SLE called Lupus Nephritis. Reliable data collected reveals that ESRD induced by LN become a really important reason accounting for increased mortality. To those with kidney problems, their mortality doubles than those without. Therefore, to figure out factors greatly affect the prognosis of LN patients.

There are lots of factors affecting one’s expectation, for instance, age, gender, lab results, pathogenic types, time of treatment, and things of that sort. However, age affecting prognosis of Lupus Nephritis doesn’t accept by all. This is because a research reports that those who are over the age of 50 years old have a poor expectation. But another research reveals that there’s no big difference between those more than 50 years old and middle aged people with LN. However, male patients have a poorer expectation than female patients. Black people have a higher incidence rate and poorer outcome. Usually, only 38% of black people survive but 68% of white ones could survive. Apparently, black patients live with a lower chance to survive this disease. This is associated with not just susceptible genes but social status and financial conditions.

Patients with a high level of circulating immune complexes, low level of serum complement and raised antibody titer are prone to suffer from progressive kidney disease. Anemia is an indicator reporting poor prognosis. In addition to anemia, declined kidney function, serious proteinuria or nephrotic syndrome is usually the indicator of poor outcome.

Kidney biopsy provides very important outcome for people with Lupus Nephritis. Membrane proliferative pathological changes often have a good outcome, and won’t progress to ESRD in most cases. Focal type (III type) is rather complicated and its prognosis can’t be told as the severity is for sure. IV type (diffuse) of Lupus Nephritis suffers from the poorest expectation among all. It is believed that result of first kidney biopsy has limited value to the expectation evaluation, but the second biopsy after 6 months means a lot instead.
