Indications of dialysis in Renal Failure

Generally speaking, dialysis will be recommended when the condition develops into Renal Failure, however, there are no absolute levels of kidney function that indicate a requirement for a dialysis, as such, the decision to start dialysis may depend on the whole body situation of patient. There are a number of clinical indications of dialysis in Chronic Renal Failure. These include:

■ Fluid overload refractory to diuretics

■ Persistent nausea and vomiting

■ Pericarditis or pleuritis

■ Hypertension poorly responsive to antihypertensive medications

■ Hyperkalemia, metabolic acidosis, hypercalcemia, hypocalcemia, and hyperphosphatemia

■ Progressive uremia encephalopathy or neuropathy, with signs such as confusion, myoclonus, asterixis, wrist or foot drop or seizures.

■ Evidence of malnutrition

■ A significant bleeding diathesis attributable to uremia

Some indications of dialysis in Acute Renal Failure:

■Severe fluid overload

■Refractory hypertension

■Uncontrollable hyperkalemia

■Nausea, vomiting, poor appetite, gastritis with hemorrhage

■Lethargy, malaise, somnolence, stupor, coma, delirium, asterixis, tremor, seizures,

■Pericarditis (risk of hemorrhage or tamponade)

■bleeding diathesis (epistaxis, gastrointestinal (GI) bleeding and etc.)

■Severe metabolic acidosis

■Blood urea nitrogen (BUN) > 70 – 100 mg/dl

For patients with Acute Renal Failure, after they are treated by the dialysis, some of cases can be even cured. However, dialysis can not cure Chronic Renal Failure. Dialysis helps patients relieve some symptoms and maintain their lives to some extent. Some dialysis patients may suffer from the complication caused by dialysis, such as headache, low blood pressure, nausea, vomiting and so on. The most important thing is that the kidney function declines after a long term of taking dialysis, that is because the kidney damage is still there and dialysis is unable to deal with it. Now more and more dialysis patients decide to stop dialysis, and that is what we want to see.
